New Patients/Insurance

If you are a new patient or your insurance has changed please bring your insurance card and a photo ID so that we can assist you in filing your insurance.

Confirm Your appointment

Did you know that we have a new automated appointment conformation system?  Please follow the prompts to confirm that you will be coming to your scheduled appointment. This system will also send out a reminder when you are past due for a cleaning or treatment. And if you are waiting for an appointment to come open, the system can let you know that there is one available so you can contact us.  




Dental problems don't always happen during normal business hours. If you are a patient of record, call our office and you will be connected to the person on call.


Payment Policies

Charles N Theobald accepts all major insurance policies and COBRA. Please bring your insurance card to your visit. In addition, we accept all major credit cards and cash. 


We want the best for you and your familys teeth. That's why we have put some information together here to help you the next time you visit our practice. 


Start 'em early

Proper oral hygiene habits are learned early.  As soon as your toddler can manage a toothbrush (24-30 months), supervised brushing instructions can create beneficial habits that last a lifetime.  Remember, children under 6 should be supervised at all times as swallowing toothpaste can be harmful.


Dr. Theobald recommends a first dentist visit somewhere in the third year.  Establishing regular visits at a young age has proven to improve the health and stability of adult teeth.


Preventative care: At least every 6 months

If you take proper care of your teeth and they are in good condition, you should come to see us for a checkup at least twice a year. If your teeth are predisposed to problems, you should be on the safe side and make an appointment for preventative care as recommened by your hygienist.
